The 500 million euro programme through which the World Bank finances family medicine in Romania gets one more year to spend the money. Through Decision no. 629/2026, the Government approved the sixth amendment to the loan agreement signed in New York on 23 September 2019 and ratified by Law no. 1/2021. The amendment was agreed through a letter signed in Brussels on 4 June 2026 and in Bucharest on 16 June 2026. It is the second act of the day concerning the financing of public services, alongside the one through which the state pays for the medical assessment of persons with disabilities.

The loan finances a results-based programme, meaning one in which the money is withdrawn as indicators agreed in advance are met, not against invoices. The objective stated in the agreement is to increase primary health care coverage for the underserved population and to improve the efficiency of health spending.

The request for the change came from the Ministry of Finance, through a letter dated 21 January 2026, asking, among other things, for a 12 month extension of the closing date for withdrawals and for the introduction of a financing component for investment projects. The Bank agreed, and the approved restructuring paper is dated 27 May 2026 and is to be published on the Bank’s external website.

What it changes in practice

The first effect is the deadline. The closing date for withdrawals becomes 30 June 2027. That is the period within which Romania may still ask the Bank for money out of the 500 million euro; after that date, the amounts not withdrawn can no longer be accessed. The 12 month extension is in fact the main reason the amendment was requested.

The second effect is structural. The loan no longer finances only a programme, but also a project. The rewritten Section 2.01 states that the money supports the financing of the programme in Schedule 1 and of the project in Part 2 of the same schedule, the two being jointly called the operation, plus support through the rapid response option. In practice, a scheme that paid for results now gains a classic line of financing for investments and technical assistance.

The third effect is the content of this new component. Part 2 has two items. The first, improving the governance, administration and cost effectiveness of the health sector, through technical assistance for the capacity of the Ministry of Health, including for health technology assessment, centralised procurement and a pricing and reimbursement system for all pharmaceutical products. The second, supporting the project management unit within the ministry, for management, monitoring, evaluation and reporting.

The fourth effect concerns who is accountable. The rewritten Article III states that the Borrower, that is the Romanian state, implements the operation through the Ministry of Health, in cooperation with other institutions. Throughout implementation the ministry must maintain a project management unit and a steering committee made up of representatives of the ministry and of the other institutions involved, and each institution must keep monitoring focal points and contribute to the reports within no more than 30 days from the end of each half year.

The fifth effect is administrative and explains part of the amendment. All references to the Ministry of Public Finance and to the abbreviation MoPF are henceforth read as the Ministry of Finance and MoF, and the representative of the Borrower is the minister of finance. Section 1.03 of the agreement is deleted in full, and the correspondence addresses of both parties are updated.

The sixth effect concerns what the money is for, which stays the same. The first results area targets primary health care coverage for the underserved population: community nurses and Roma health mediators hired and trained to work in communities, health education and navigation support for vulnerable groups, plus a minimum benefit package for uninsured persons, aligned with the basic benefit package. The second area targets the shift of emphasis from the hospital towards primary health care, by revising the benefit package, strengthening prevention and correcting the payment mechanisms of the National Health Insurance House.

What has changed compared with the previous situation

Compared with the previous form of the agreement, the substantive change is the dual nature of the loan. Until now it was a results-based programme, in which withdrawals were made when agreed indicators were met. From now on, alongside it there is also a project financed in the classic way, for technical assistance and management. This is an implicit acknowledgement that the indicators alone were not enough: to meet them, the ministry needed technical support that the initial scheme did not cover.

The second change is the timetable. The closing date for withdrawals moves by one year, to 30 June 2027. The schedule of disbursement-linked indicators keeps results that are to be achieved in 2025, 2026 and 2027, and the text specifies that the indicators may also be met up to the closing date for withdrawals.

The third change concerns the legal architecture of the documents. Schedules 1, 2 and 4 to the loan agreement are replaced in full with the texts in the annexes to the amendment letter, the addendum to Schedule 2 is deleted in its entirety, and the attached appendix, the one with the definitions, is rewritten as well. In other words, anyone consulting the agreement must now read the version in this issue of the Official Gazette of Romania, not the one in Law no. 1/2021.

What has not changed: the amount. The loan remains 500 million euro, with the possibility of periodic currency conversion. The amendment does not add funding, it redistributes the way it can be used and extends the deadline.

Advantages and disadvantages

What it improves

  • The money stays available for another year. Without the extension, the amounts not withdrawn by the previous deadline would have been lost, and the programme would have stopped halfway.
  • The new technical assistance component addresses precisely the weak points acknowledged in the agreement: health technology assessment, centralised procurement and a coherent pricing and reimbursement system for medicines.
  • The implementation structure is strengthened, with a project management unit and a steering committee that must be maintained throughout the operation.
  • The objective stays focused on family medicine and on underserved communities, including through community nurses and health mediators.
  • The restructuring paper is to be published on the World Bank’s website, so it will be open to public scrutiny.

What remains a problem

  • An extension requested more than six years after the agreement was signed shows that the pace of implementation has been far below the one planned.
  • This is the sixth amendment to the same agreement, which makes the version in force hard to follow: the text in Law no. 1/2021 no longer matches reality.
  • The schedule of indicators keeps results that were due in 2025, so part of the delays is carried forward rather than made up.
  • The amount does not increase, so everything added as technical assistance is financed out of the same 500 million euro that was meant to go towards results.
  • The Government decision approves the amendment, but its entry into force depends on an exchange of notifications with the Bank, that is on a step that is not visible in the Official Gazette of Romania.

Practical advice

  1. If you work in an institution involved in the programme, note the reporting deadline: the contribution to the programme reports is sent within no more than 30 days from the end of each calendar half year.
  2. If you follow the financing of primary health care, the date that matters now is 30 June 2027. After it, the amounts not withdrawn can no longer be accessed.
  3. Do not read the agreement in Law no. 1/2021 as the version in force. Schedules 1, 2 and 4 and the attached appendix have been replaced by this amendment.
  4. Health care providers interested in the minimum benefit package for uninsured persons will find the objective described in results area 1 of Schedule 1, in the version published now.
  5. For those following procurement in health care, the new technical assistance component explicitly includes centralised procurement and a pricing and reimbursement system for all pharmaceutical products.
  6. The restructuring paper, dated 27 May 2026, is to be published on the World Bank’s external website. It is the source that explains why the restructuring was needed.
